Practice Update

Life's journey takes us to sooner-than-expected forks in the road. Due to unexpected circumstances, after much deliberation, it is with a very heavy heart that I have decided to close my practice and focus on my personal life.

Please be assured that I will be available to finish treatment in progress and I will be available for emergencies until December 2nd, 2024.

For new provider-Please contact your insurance or you can contact the Sacramento District Dental Society at 916-446-1211 for a referral.

To facilitate the change to a new dentist, we will make copies of your dental records available to the dentist you designate. Because dental records are confidential, your written authorization is required to make them available to another dentist. Please contact us to complete your authorization to transfer your dental records to another dentist.
